In some cases, yes. If it's something like birth control a person can go to a planned parenthood and get it without parental consent if you are 13 years old. You can't have an abortion without parental consent until age 16.

I know that some places have to keep information confidential. Kaiser Permanente on the west coast is. Even if the insurance is provided by a person other than the patient, the hospital cannot disclose any information to anyone other than the patient. I believe Planned Parenthood is the same way.

I do believe so. Apparently it goes by state laws. The ACLU page has a list of guidelines to help health care workers determine if the parents need to be or can be notified in certain situations.
I think really it doesn't come up very often cause most of the time a parent is with them when they go to the doctors. Also most young people wouldn't realize that all they have to do is request their parents to not be notified. I think once a doctor says we need to call your parents they just figure that's the way it is.
But most states have a separate law specifically for abortions and you usually have to have one if not both of the parents notified (not necessarily consenting).

unfortunately they don't, because if there were signs of child abuse or molestation the doctors have to report to the parents, no doctor-patient privileges there.

no, minors don't pay bills; and depending on the age and maturity level of the minor they definitely aren't able to make life/death decisions. (if more were, they'd keep their legs closed!)

Absolutely not
