It depends on what you've seen, but... here are some of the scariest I've seen:
The Exorcist
The Entity
The Ring
American Werewolf in London
Stir of Echos
That's all I can think of that's REALLY scary. I usually read horror instead of watching it because it's generally scarier to read a horror novel than to watch a horror movie.

The Evil Dead Trilogy are classics. 1 & 2 and Army Of Darkness. You gotta see them all at once in a row for it to work right. The first is really independant, but remember this producer/director, Sam Raimi, later made Xena and Spiderman (and is a mastermind behind a lot of these japan horror remakes).
I'll assume you've seen Halloween...see Black Christmas, it's a forerunner to halloween. It started the whole slasher genre, and is a really scary film too.

DON'T watch Saw, The Grudge, The RIng or White Noise. Pointless. The Village has a weird ending. I heard Lady in the Water is good (in theatres). I liked Thirteen or something from a long time ago. 13 different ghost things.

I liked Resident Evil and Resident Evil: Apocalypse. "Fright Night" is always good for a few laughs. Some older ghost mystery thrillers "Lady in White" and "Ghost Story." Don't waist your time on "The Ring", or "The Ring 2".
"From Dusk until Dawn' is Robert Rodriguez's funny take on vampires.

gah i don't know...
i'm obsessed with horror movies and to tell you the truth new ones wont do ya that much good seeing as they're getting less and less original now and i'm disappointed with a lot of what the filmakers are putting in them.
it seems they're more about sex than the actual scary stuff.
idk but the world is dissappointing
