From the Social Security website:

You can replace your Social Security card for free if it is lost or stolen. However, you are limited to three replacement cards in a year and 10 during your lifetime. Legal name changes and other exceptions do not count toward these limits. For example, changes in noncitizen status that require card updates may not count toward these limits. Also, you may not be affected by these limits if you can prove you need the card to prevent a significant hardship.

To get a replacement card:

* Complete an Application For A Social Security Card (Form SS-5); and
* Show us documents proving:
- U.S. citizenship (or immigration status and work eligibility); and
- Identity.
* Take (or mail) your completed application and documents to your local Social Security office.

All documents must be either originals or copies certified by the issuing agency. We cannot accept photocopies or notarized copies of documents.

Identity - Your child: We can accept only certain documents as proof of your child’s identity. An acceptable document must be current (not expired) and show your child’s name, identifying information and preferably a recent photograph. We generally can accept a non-photo identity document if it has enough information to identify the child (such as the child’s name and age, date of birth or parents’ names). We prefer to see the child’s U.S. passport. If that document is not available, we may accept the child’s:

* Adoption decree;
* Doctor, clinic or hospital record;
* Religious record (e.g., baptismal record);
* Daycare center or school record; or
* School identification card.

Good question. I don't know, but I'd hazard a guess that since he already had a social security card, you could take the kid and his birth certificate to the local Social Security office and fill out the form to request a replacement card.

Call the Social Security office to verify the requirements. Make sure you take with you:

1) the child

2) school records, such as report card

3) birth certificate and baptismal certificate (if applicable)

4) immunization records

5) anything else that establishes the child's legitimate birth as a US citizen.

These kids are getting jobs younger and younger, I guess!

Take the child's birth certificate to the Social Security office. You have to do it in person. Probably take a number and wait in line. But it can be done.

Contact you local Social security office and have them issue a new one it will cost about 20 to 25 $ though.

Go to your local social security office and make sure you have ID and birth certificate. Or you can download the form off of their website but you still have to go to the office.
