It means that they pitch as if it were a real game, but usually without a batter. They'll be pitching to a catcher, usually with a coach calling the game - that is, the coach will say things like "the batter just hit a double to left-center" or similar things, and the pitcher will be expected to pitch as he would if it were that situation in a real game. The advantage is that the pitcher is forced to change his style and delivery as he would in the various situations he'd encounter in an actual game.

It's when a pitcher is rehabing from an injury, get pitches to like 4 of his teamates, so they have good batters, and they test the pitcher to see if he's ready to pitch a real game.

A simulated game gives you the advantage of being able to pitch off a mound, in the bullpen. You throw your pitches and the bullpen coach calls balls, strikes, hits etc. It gives the pitcher the mental practice to understand how he should pitch in different counts and situations without actually having to do it in a game.

It's like practice for the rest of the team. It usually happens when they are coming back from an injury to see if they are doing good enough to start. There ar no batters.
