If I had to put in a 'period,' I do it like #1. It's the question mark that is screwing me up.

punctuation goes inside the the quotes doesnt matter if its question mark,period, exclamation etc

Question mark goes inside the quotes, AND a space is needed between the quote and the paren....but consider what you're trying to do. This is worded more like a Movie Trailer than a footnote or option. Try something more like this:

For further information on events, see appendix A.
or
To see what happened, see appendix A.

it depend on context-

if , lets say, a character in a novel asks a question then the ? goes inside the quote because in this case it is part of the statement
