I would say 9x out of 10 it does. I wish they would just say no and get it over with.

if it involes a relationship or date it almost always means no, but in other contexts it can really mean maybe. such as "hey a bunch of people from (insert team/club/group of friends) are going to see (insert movie) at (insert time). you wanna come?" you respond with "maybe, i need to ask my parents first," or ,"maybe, i might be going out to eat tonight."

By begining with one expressions people use very often which "I Think" wich means that "You Don't Know" , I would say that "Pretty Much" always means "No".
