How can we help the homeless better locate the resources/programs that are out there to help them?

I chose to post this question because of the responses I received from my last one. My last question went something like "Why does America rush to send food and supplies to other countries when there are people here who are in need?"

Send out volunteers on foot to consult with homeless individuals and arrange transportation for them. So many times it's not the lack of knowledge of resources, it's the lack of a way to get to them.
