It depends on the sites and the target audience.

Vonage is paying as low as $0.87 per CPM in their campaigns in banner ad networks such as Valueclick and Burst while Lane Bryant pays higher at $3.97. Some sites charge $5 CPM -- if they have a really targeted audience that is not as easy to reach

The average CPM nowadays seem to fall between $1.20-$1.80 per CPM

But if you are seeking out specific market that is harder to reach or the site you want to advertise is top notch, CPMs can go up as high as $5-25. Mapquest, for example charges $8 CPM (for national) and $9.60 (geotargeted) in their banners for results pages http://www.mapquest.com/mediakit/ratecard.htm

That depends.

Some places charge thousands. Just contact the webmaster.

What is it you are advertising? I have a site and not very many buttons on it.

Some sites will give you an affiliate link if you put one on your site.
