I'm in the process of having one removed.
They use a laser and go over it essentially burning the tattoo off. The laster makes the ink molecules explode, and your body carries it out like waste.
I'm going on having my 5th treatment, and although I do notice a difference, it's nowhere near gone.
It's a very expensive, painful procedure even though it only lasts a couple of seconds.
The last treatment I had they had the laser on way too high, and now I have a 3rd degree burn that hurts bad.
I can honestly say I would sit and think LONG and HARD before I EVER have one removed again.
Also, if you decide to have one removed, go to a plastic surgeon, DO NOT go to a laser clinic. The laser clinics for the most part are not staffed by doctors like a plastic surgeon, just people who are trained to use the laser, which means, if they burn you, like they did me, you have to go to a different place to be treated, and incure those medical expenses. Also laser clinics in general specialize in hair removal, microderm abbrasion, etc, not removal of tattoos. I was dumb, and went to one because they were having a deal on tattoo removal.
A plastic surgeon is a lot more expensive, but if you decide to have one removed, you want it done the RIGHT way.

"Laser"
I've been with someone who had it done. Its very painful, and you have to go several times depending on if you have color and how good the tattoo is. She said it was worse pain than getting the tattoo.
