If they are having a heart attack or something, they cant be held accountable for not taking medication, as they may not be capable of doing so if they are in an emergency situation.

No, they're just letting nature take its course. The reason why they do this is probably their ethics on medicine, not because they're unhappy with themselves and want to leave the world.
