I have snuck things into his wallet (pics, notes, etc). I have also put things in his back pocket (where he keeps his wallet). I have put things in his lunchbox, etc. I have even taped a pic of me onto the face of his watch.

I named a star for mine for our anniversary and then packed a picnic and we went out in the middle of the night to the beach to look for it (you could go anywhere really if you do not live near the coast) sounds a bit naff, but it was a really sweet night, hope this helps x

I just surprised my hubby this weekend...it was our anniversary. I snuck out of bed, made him breakfast in bed, and then surprised him with a trip to Boston... it was great...
I've also just bought little things that I know that he likes (i.e. little chocolates or candies) and have hidden them in places where I know that he will find them. I'll leave a little note just letting him know that I was thinking of him and that I love him. He travels quite a bit so it's kind of fun to sneak things into his luggage.

If kids, by making special times out for just the two of you and your relationship.
Actually, thinking about it, the same rule applies regardless of children.

Redo your whole outside appearance, hair style, makeup, clothes, and your attitude. Talk to him like you did when the two of you first met. There was something there then, that's why he chose you over others. Rekindle the old flame.

I would ask him to get dressed and don't tell him where you're taking him.
Even if he begs, don't tell him!!! When you get to the restaurant or movies, then just say you wanted to surprise him.
