Design your own and print an "original" - if there are colors, create a B/W copy also for the carbon pages. Go to a local printing company (eg: Kinkos) and they will duplicate using carbonless yellow / pink copies. They will usually keep your original on file for future orders. They will also cost LOTS less than custom orders.

I would just call places in your area first. Like any some kind of office supply stores and maybe even places like kinko's that are mainly for printing and copoying things for customers. If they don't do it themselves just ask them if they know anywhere - that may be your best bet.

As others have suggested before, I would seek out a decent printing company. Most of them have graphic designers on board who specialize in forms, business cards, invoices, etc...I would try this: http://www.google.com/search?hl=en&q=Print+Shops&btnG=Google+Search and then localize your search from there.

Call Paragon Print Systems at 1-888-237-8525, or visit http://www.printparagon.com. They can design all kinds of custom forms for you, or print from your design, and set up a regular system for reorders and such.
