Yes, most failures occur because people don't use the condom properly. It must be in place before there is ANY contact between the male and female genitals. Condoms can tear if you are particularly rough with one another, and the penis after sex (and the condom is removed) will still have some sperm in the tip, so if that comes in contact with you, you may get pregnant that way too.

Condoms are very good at reducing disease transmission, but they are not as good at prevent pregnancy as the pill.

yes. many a child has been born by way of a condom that seemingly worked just fine--as in, it didn't break, didn't seem to leak, but suddenly the woman's pregnant. It happens. Microscopic tears in the condom. I've never had it happen; my husband and I have used them for our sole form of birth control for years and I've gotten pregnant when I want to (and I'm very fertile).

Condoms fail to prevent pregnancy somewhere between 10% to 30% of the time. So yes, you can be pregnant.
