No. The whole point of it is so they don't have to provide benefits.

No, usually not. But sometimes contract workers get hired on by the company and then benefits will start.

Nope. I signed a contract to do free-lance writing for a newspaper. One of the downsides of "contract" workers is usually no benefits.
