Sex. That's the key, Sex in some form or the other.
Unless you are one of the lucky few who are reproducing parenthetically.
If it was because of sex, and you were using protection, it could be because of a break or tiny hole in a condom and you must remember that condoms are only 96% effective.
If you are on the pill, you must remember that the pill is only 98% effective. And, if you were on the pill, you must also remember that if you take many medications, from antibiotics to pain killers, the pill does not work! That information is included in the information sheets that come with the pill but lots of people don't bother to read it. Several of my daughters have been caught out by this, even though I pointed it out, years before, and each daughter told the others, and they still forgot and got pregnant on the pill.
Sad world we live in when people don't read the information.
Or Immaculate conception?

Sorry, there's still only one way--I only know of one case where it wasn't, and that was 2,000 years ago. Also, birth control isn't foolproof, if that's what you mean.
