Make an affidavit and have it notarized showing that Mrs. Caridad M. Alcaraz is the same person as Mrs. Caridad M. Alcalas. The said affidavit and the birth certificate should be presented in all previous documents filed with the said names to correct any entries for purposes of insurance, employment and others.
