I served with the Marines in Khe Sanh and in the DMZ in 1968 and each movie had pieces which were good, but I thought Platoon was the most realistic in displaying the way human emotions can be misguided into evil deeds which in a war scenario can often go unpunished. But from a technical point of view, Saving Private Ryan (a WWII movie) was the most realistic war film I've ever seen. The visuals and sounds of the bullets and explosions had never before been so authentic.
Apocalypse Now is more of a pyschological remake of Conrad's Heart of Darkness and just tried to make Americans look ridiculous. Some scenes, like the one with the helicopter attack and the Wagner music did ring true, however.
One is waiting for Spielberg or his equivalent to tackle this subject.

I would have to say Full Metal Jacket, followed closely by Apocalypse Now and in third ('cause the people never actually made it to Vietnam) Tigerland.
