After that there are scenes that really let you know that it IS one year later....on the Galactica, Adama now has a 'stache, Lee's put on some weight and Dualla is sporting a 'tude. (She's still with Apollo) Meanwhile back on the planet Baltar forced them to settle on...Kara and pyramid player superstar/resistance fighter are now married. He has pneumonia, though, so you get a nice scene with Doc Cottle letting them know that there are no meds available, but MAYBE he'll make it. So Doc Cottle's maybe the only one who's still the same. He suggests that Kara call Lee--and thats where you find out that something bad went down during that fast-forwarded year between them. Kara's hair is also down to her waist, which may or may not be significant in the new season.
Meanwhile you see Tighe and his wife strolling about and they happen upon a union rally led by Tyrol and by his side pregnant ... Callie? We also see that Roslin's a schoolteacher again, but her aide is the woman who adopted Hera. Hera is shown briefly, looking pretty much like she's about 10-12 months old (she was a preemie after all)
There are a few other people who show up...including someone looking for Kara. Lets just say its not really someone she'll be happy to see, and definitely a plot thread for the new season.
But of course the cliffhanger is what you really want to know about.
Lt. Gaeta is now Baltar's Billy, and what you see in the final scenes are Cylon ships across the new planets skies. Uh-oh, so much for being perfectly hidden. Sure enough, you see Six, Sharon and Male Cylon whose name excapes me entering into Baltar's office. They let him know that they WOULD'VE missed this planet entirely, if only they hadn't seen a nuclear signature one lightyear away. Yes, THAT one. It dawns on Baltar that's the explosion that Six/Gina had set off, killing herself after having sex with him (exactly as it sounds.)
The very last scene is of the Cylons marching through the settlement, a determined Kara vowing to "Fight 'til we can't fight any more" and the rest of us wondering how we're going to make it until October when the new season begins!!!!

Um.... SPOILER ALERT!
The Cylons fly down, and Six and some other Cylons explain they found the fleet from the residue of a nuclear bomb. Galactica and the rest of the still orbiting fleet bug out.
