check closely all the other pulleys run by that belt. maybe a bad bearing or something not aligned right. if all okay, get a new tensioner.

If the belt is, or was, in good condition, then you'll have to check the alignment of the pulleys. If they are where they are supposed to be, then you probably have a bad bearing in the tensioner pulley, or the idler pulley. A higher walled one won't make any difference. As long as everything is lined up and working correctly, you can run a pulley with no sides on it. The belt will ride on the pulley just fine.

if the belt sliped half way off pulley then there is a problem with the belt or it is not in the propper position on all the pulleys... that is the only way it will come half way off any pulley, or the tentioner or idler pulley may have bad bearings, or waer pump is going out an has a wobble in it....... 30 yrs mech.

Before you purchase a new belt and install you should look for the cause. Misaligned components can cause this problem. If you look and find nothing wrong replace the bet but only after you have inspected.

Try putting it back on and see what happens. If it slips off again put a new one on
