ALL of them! but a toothache can kick your azz worse than the other too! I hate toothaches!

As far as I know, the earache can be the worst.
Because the facial nerve (nervus facialis) is quite exposed in the ear area and any ear infection will very likely affect the facial nerve - easier than in case of a toothache or a headache.

Of course any bad infection can affect the facial nerve. An infected trigeminus nerve is the worst and includes all the toothache, earache or headache in one big pain.

When I have a bad migraine I use a little towel, wet one end and roll the towel that the wet end is in the center of the roll. Then I heat it in the microwave for 1 min. I apply it for a few seconds on the head, usually one side, then take it away and redo this a couple of times. This helps to improve the blood circulation and the pain gets immediately better.

But if you have anything infected, be it tooth or ears, never use heat. It makes everything worse. Instead use ice to calm down the infection, then go and see a doctor as soon as you can.

Toothache!! Cause you gotta go to the dentist to get it fixed!! They always make you feel worse before you feel better. A headache you just take some advil and you get better and an earache you go to the doc and they give you meds no more pain there! LOL! Toothache you have to either get it pulled, drilled, or root canaled!

I have had all three, from migraines to swimmers ear. And I must say a toothache is by far the worst and most painful, especially if it is an abscessed tooth and you have to wait to see the dentist. Sometimes, even with anti-biotics, and aspirin the toothache will not go away till you see the dentist, and he does a root canal. This leads to other pain. Therefore the winner is toothache.

For me it's a toss up between a molar cavity and a swimmer's ear infection...
The toothache is a stronger pain, but is easily relieved quickly, painkillers and dentist. Ear aches can stab into the jaw and make your life miserable, and that takes days to go away even on meds. Meanwhile, you can't chew, open your mouth or sleep on that side....

Earache, if it's bad enough it will feel like a toothace and give you a headache so it would feel like you've got all 3!

I have seen grown men laid out on the floor over a toothache. So, I think this is probably worse.

all 3 can be pretty bad but if I had to choose one it would have to be a toothache.

I would say headache. I've had some very bad migraines. I once had a migraine so bad that I was seeing blurry spots and half of my body went numb. I couldn't remember my name, and my speech was slurred. My mom and dad took me to the emergency room, and while there I vomited, which somehow got rid of my headache. They said it was a migraine.

If a headache is caused by a stroke or a severe blow the headache is the worst.

There is no way that a toot or an ear can hurt as bad as this example.
