Honesty. I would expect nothing else. No matter what, if your friend, wife, girlfriend whoever doesn't want to hear your answer then tell them not to ask. If your gonna ask a ? then be prepared for a answer you may not like, cause a true friend/lover will be honest and they shld understand that.I wouldn't want to go out or have someone i like go out looking like a fashion drop-out.
And yes their are clothes that will make a person look larger or thicker shorter or taller whatever, all depending on the style of the clothes and the persons shape and size

No, it's your fat that makes you look fat. Let's face it, the [piece of clothing] without you in it, looks flat. Practically 2D. It's when you put your fat in it, that you look fat.

I would strongly suggest you say something like "No honey, you look wonderful"...sounds like fishing for a compliment.
