It means to think you are better than someone else from the habit many years ago of better off people sticking their noses in the air as they walked past beggars and so on. It was a symbolic gesture denoting that the poorer person probably hadn't washed and thus smelled badly. The term became 'Stuck Up' to describe anyone wealthy or with delusions of self grandeur.

The term derives from the practice of aristocracy looking down one's nose at the common masses. They would hold their head back when approached by "unpolite" society. Stuck up is short for having one's nose stuck up in the air like the aristocracy. The term is used for anyone who believes they are better than others.

What I have observed is, that is you got a pretty face and dress in the best clothes and got the money you look down on others. Especially non attractive people by not even speaking to them.
