Shankar, Principles of Quantum Mechanics. This book's worth buying for the mathemantical introduction alone. I don't know if I'd call it "elementary". It's a real text book. It starts from the beinging, though, and it's not too scary. Schiff, Quantum Mechanics, is scary.

As for Quantum physics, there is a greatly underappreciated writer named Paul Davies. His book "Other Worlds" is a mind-blowing introduction to quantum physics that never fails to bring up interesting ideas while making them easy to understand. It's also really cheap. Here:
