No she doesn't have implants but I think she has had something done to her breasts, probably some augmentation because her breasts are not small and not big, but different looking from when she was younger.
And to those who are saying she is "perfect", newflash, there is NO such thing as a perfect person let alone a perfect woman, believe it or not Monica Bellucci has her flaws like the rest of us.

Heyy. What's up? My name's Gwenivere, let me tell you a brief story about myself which I believe may be of some Hẹlp to you... Growing up, I was teased all throughout high-school. You know, I was that girl with the really small breasts that everyone liked to make fun of. I did not hit puberty until a little bit later on in my life. you cannot do just "a" breast implant--you gotta do a pair of breast implants so as to equal things out.
Who would want one nice firm full nippled breast and one flat rancid crappy looking breast as a "pair"?
And If she does, she most certinally deserves to have whatever she can afford.

No she does not, she is perfection, I doubt she had any plastic surgery, is she did he is flawless because everything appears natural about her.
