They're going to the lands of the Valar, far in the west. Frodo goes on the boat, because he has carried the ring, which means that he won't be able to find rest in Middle-Earth anymore. All the ringbearers; Elrond, Gandalf and Galadriel go on the boat. If you read the book, you'll find all the answers. You could also read the Silmarillion, where everything is being told, from the beginning of the earth of Tolkien till now.

Frodo feels like he's been through to much to go back to the shire, he wouldn't fit in because the ring changed him to much. So he goes with the elves, all of which are leaving Middle Earth, to thier original homeland across the sea.

They are going to the undying lands, home of the Valar. They are doing this because they have earned the right as ring bearers. And he will not die there.
