Since I am the type of person who likes practical jokes and most of my friends would expect nothing less than something sarcastic.. I would get him a dozen dead roses or something that would make them laugh.. (gag gift). I guess IF I did have some serious type of friends .. for a male. I would get a gift certificate to a home improvement store or one from a clothing store that he frequents.

Unless he has a great sense of humor and isn't stressed by the big 4-0, then no gag gifts!!!
Why not just take him out to dinner or somewhere else he might like? It doesn't have to be a wrapped gift, you know. Think of what he loves to do (no, not sex necessarily!) and treat him to that: a concert; show; a round of golf. It really depends on his personality/likes/dislikes.
If you know him well, then figure it out or else ask him!!

Go to the store that sells OVER THE HILL products . . Put them all in gift bag and buy him a walker , decorate the walker with ribbons and hang the gift from the walker . I got this for my 40th and loved it . For my 59th , they bought me a cane with rear view mirror , bicycle horn , change purse , etc ... still have ALL of them too cute to get rid of ....

Take him out and buy it together! Unless you don't know what he likes. I bought a friend a real silver keyring with a bit of real gold on it too.... a beauty..
listen to what a wrote one the card...
Life is like the silver keyring ... broad with loads of opportunities...
But there is only a thin layer of real gold ... you have to make choices to enjoy the real fruits of life! for as a fruitful year lies ahead of you! the gold is just a thin layer ... make sure not to overlook it..
