He should not fly with a middle ear infection, as he will be more prone to "barotrauma" of his ears, that is a painful condition when the eardrum becomes stretched by the changing ambient air pressure inside the aircraft cabin, causing bleeding and very painful eardrums. (More likely on landing as pressure changes are greatest then.)
We need to look at the swelling behind the ear separately as the diagnosis is not clear. It can be a swelling of back of the pinna (outer ear) or a swollen lymph node or even a mastoid bone swelling. All requiring treatment.
Advise would probably be best from an ear, nose and throat doctor, after careful examination.

A problem with the ear specifically the inner ear sounds to me to present a problem with flying due to changes of any kind in pressure. my suggestion is for you to talk to a doctor who specializes in the field of ear nose and throat conditions. I also suggest that you not take advice from laymen, you do not want to put your child at risk by following well meaning , but dangerous advice. this question is for the professionals.

It's likely to cause him a LOT of pain due to the pressure changes that occur in the middle ear during take off and landing.

I would not. I have very fussy ears and one time, I had a minor infection and a cold and I flew. I felt as if my head would burst. The pain was UNBELIEVABLE. He will be screamin his head off. Just don´t.

You should ask your doctor to be sure but I would say it shouldn't be a problem unless there's a breach in cabin pressure which is highly unlikely of course.

omg..........ouch. all i can see is your son in more agony from the altitude change and his ears popping. i say it will hurt him alot. but may not make it worse.
