Sigmund Freud
He was convinced he knew what motivated everybody's behaviour, including some really ridiculous sexual ideas.
And he was the first to coin the term "ego" in the context that you are using it (since it just means "I" in Latin).
Hitler - clearly an egomaniac too, but given that power I think there are a lot of people who have similar egos who may have done the same - Pol Pot would have killed more if he could, so would Idi Amin, both equally egomaniacal. I also don't think Hitler was more egomaniacal than Stalin or Mao, who have actually probably been responsible for as many deaths.

I am not sure I agree with the choice of Hitler, and rue the fact that no one has yet listed Stalin.
Sure Hitler had Goebbels and Riefenstahl as media workers, to promote that cult of personality, but directed the people towards the German People, and the promotion of the Third Reich.
Stalin was an abomination, and at least as equally horrible as Hitler. Stalin did work to promote Stalinism, and destroyed more competent people to promote himself.
Both of them were seriously flawed individuals, extremely manipulative and horrific. Genocide, horrible as it is, does not equate to ego. Especially in Hitler's case, since he was probably part Jewish, and his genocide was self denial.
Napoleon Bonaparte would be an interesting choice. He crowned himself Emperor while the Papal delegation was ignored. Plus, after he was beaten, and discharged to Elba, he returned to stir up so much trouble he had to be deported to St Helena, a LONG way from Europe. Not bad for a fellow known as the little corporal.
Nowadays, Rush Limbaugh is a serious contender for biggest ego in the history of the world.

The question is subjective, of course.
I have three candidates:
Nebuchadnezzar. His ego got so big that God struck him with
a rather severe mental illness--thought he was a grass
eating animal--) for a while. But he came out of it, so I think No. 2 has my vote.
The Pharoah of the Exodus. It took ten plagues which were absolutely destroying his country before he was willing to let the Israelites go, including the loss of his first born son, and then as soon as he'd done it he regretted it and went after them. Anyone who'd experienced the horrors of that much power thrown against him and still thought he could beat God has my vote as having a huge ego. Unlike Hitler he was in a direct contest between his own will and the will of the God of the Hebrews. So he gets my vote.
My third thought was Alexander, but as I think of it, he doesn't come close to Pharoah.

The country of France. As a whole, they all think they have the world's answers but can't see the problems in their own country or the problems that would be caused if the entire world took their ideas to heart. I'm all for loyalty and even ethnocentrism, but do they really believe they are as perfect as they profess? So if you are looking for one individual person, I'm not certain who it would be, but they'd have to be French.
