Wheel bearings wether tapered or ball bearing type wear from use. If they are damaged then either foreign material was introduced into the bearing and caused the damage or they wore down so much that they came apart. Cost to replace depends on the vehicle, if it is a front hub assembly you could be looking at anywhere from $50 to $350, for just bearings they are around $10.00 -$25.00

Normally caused by some sort of suspension problems causing the wheels to turn in or out it will grind the wheel bearings and not ever packing them doesnt help either.

Over time grease leaves the bearings, then they get rough, and make noise, though passing through high water, water gets in and makes rust. Another reason is tires out of balance, the car's suspension shakes, either front or rear, and helps to finish the bearing earlier.
