No, it doesn't hurt, per se, but it's a rather weird feeling. First off, as you're sticking your tongue out to get prepped for the piercing, your tongue will get tired. No pain there, just a non-normal feeling (experience it yourself! stick your tongue out for about 3 or 4 minutes). The clamps don't hurt either, no more than you holding your tongue out with your thumb and forefinger. That there, in my honest opinion, is the worst part of it (having it stuck out there for so long).

The actual piercing is no more painful than having your ear pierced, just a little pressure and a pinch. However, you can feel the needle go through your tongue, and although it doesn't hurt beyond that initial pinch, it is a truly weird feeling. Then sending the guidewire through doesn't hurt either (it has the same feeling as if you get a long hair stuck on your eyeball, and as you pull it off, you feel it drag across your eyeball. Not painful, but not exactly pleasurable either). During all this, your tongue is getting even more tired from being stuck out. Putting in the barbell is nothing at all, and then you're done.

Your tongue will immediately feel somewhat numbed, and you'll speak with a bit of a lisp right away (same effect as getting novacaine from a dentist, though the numb feeling is very mild), however the lisp'll go away when the swelling is gone and your barbell is resized.

It will start hurting several hours later as the swelling kicks in. If you've ever bit your tongue pretty hard (but not so hard as to draw blood), you know the feeling. Let ice chips dissolve on your tongue, drink ice water, eat ice cream and popsicles, and other cold things to keep the swelling and pain down. Also, take an anti-inflammitory medication (ibuprofen or Advil) to also fight off the pain and swelling. Avoid spicy or acidic foods until Day 4 at least, as this will hurt something horribly, and cause the swelling to go right back up again. Also, avoid chewing. Since your tongue now takes up a good portion of your mouth, it's really likely you'll bite your tongue or barbell (I did, repeatedly), and that's none too fun either.

During this time, talk as little as possible. Your tongue will still be too tired to actually *want* to talk, plus talking will increase the swelling (and you can feel it swell the more you talk). The pain will last, as a constantly annoying throbbing, the first day. The second day is a dull throbbing, and at times, will be unnoticeable (or have a few little flareups making it feel like day one again). Day three, the swelling is gone, flareups are minimal (usually only happens if you bite your tongue and/or barbell), and don't hurt that much, and the constant lisp is gone (although certain sounds are now hard to pronounce as the now-oversized barbell is flopping about in your mouth. "S", "L", and words than began with "Th" were hard for me, but I found ways around that at this point).

Once you get your barbell resized a week later, having it inserted will put you back to Day 2's worth of pain, but it only lasts for a few hours, and by the end of that day will be like Day 3 again. The next day, it won't hurt again if you keep it free from infection. You also won't have to hear that horrid lisp again from now on.

Umm. . let's see. . . You're having a major (and very sensitive (and pain-sensitive) organ of your body cut through by someone.

You tell me. Do you think that will be a picnic? A party?

I would never have anything pierced cause I think it's stupid, but I certainly wouldn't tell anyone that it's gonna be painless, especially something as thick (and sensitive) as the tongue!

i had it done and it did not hurt at all. piercing my ears hurt worse than my tongue did. drink only ice cold water for a few days afterwards and take a cup of ice with you to suck on afterwards. it helps keep the swelling down.

No, having your tongue impaled with a large needle thing is completely painless. Absolutely.
