don't want out of the box php apps, need something built for speed and volume. what do the other big sites use? flickr, youtube etc? db2 or oracle? do the big db systems have modules for galleries etc?

In that case, all you needed to have is a huge storage drive to store the images (very high speed [rpm]) and a huge Bandwidth.
Also, fiber optics would be recommended for such a load and
maybe, split up like the first 200,000 photos in one server ,then the next 200,000 photos in another and so on. Just divide the load
to each servers.
