There is no such issue. What I believe you're thinking of is Mark Grunewald's (another Marvel writer/editor) last wishes to have his ashes mixed into the ink of the Squadron Supreme trade paperback. This only occurred in the first printing, so they're difficult to come by.
If you haven't read Squadron Supreme, I strongly suggest you do so. It's great.

I am pretty sure Steve Ditko is still alive, so I really hope his ashes are not in an issue, because that would be really reallly creepy. According to this, he is still alive as of this year and living in NYC. He does not give interviews however, and has not for the past 40 yeras. Other artist have put their ashes in comics, but they usually wait till their dead.

While that would be cool, it wouldn't happen for two reasons:
1. Ditko could care less about Spider-Man today and wouldn't particpate whatsoever.
2. He ain't dead.

But I know Kiss had their blood mixed in with the ink in Marvel's Kiss Comics #1. Really!
