I'd hazard to say most directors simply make the story from the script handed to them, fleshing out what existed in the writer's mind so that we can see and hear it for ourselves. So what we see on the screen is really just the director's interpretation of the writer's story, rather than their own imagination.
That being said, I'm sure there are times when horror directors (and the rest of the crew, for that matter) are haunted by what they create. It's their vision of the story, and as is the way of visions, they have a way of coming back to you at times. But they're not mentally ill; they simply have found their niche, same as the guy who can always be counted upon to tell the scary story around the campfire. They do it because we listen (or watch, in the case of moviemakers). We listen or watch because, on some level, we like it. It's a thrill to us to see and hear something that we would otherwise never experience (nor want to... getting my brain dug out from my nose with a grapefruit spoon isn't high on my list of things to do in life)
They are a little weird at times, but they're artists, who can all be weird in one way or another. Doesn't mean they're any stranger than anybody else, only that they tell stories.

I don't know about sick in the head although a fair amount are addicted to drugs and booze. Movies ( and TV) are all about making a profit. In a modern version of throwing the Christians to the lions ( which always drew a crowd) you put together the most outrageous, sensational thing you can think of together with something that will make people cry, and then you sell it to the masses. That's show biz! This is why, for example, I do not plan to see the Oliver Stone film about the Twin Towers. He is exploiting some peoples pain in order to give other people a vicarious thrill. I do not care to re-experience that day. I have the same opinion of some other films that were both widely seen and hyped. Its all done for money, don't you know?

Some of the best horror writers of all time have been really screwed up in the head or substance abusers. Arguably the best horror writer of all time, Edgar Allen Poe, was an opium addict. But not only horror writers are screwed up in the head, as directors go, Kevin Smith is out there but not nearly as out there as Woody Allen. But that's just my 2 cents. They're just being themselves. Most directors aren't serial killers and don't even come close. People who have directored movies like Friday The Thirteenth Series just had a really good idea and went with it.

I don't think it is mostly the directors. The writers have more to do with the sick stuff because they write most of the story and outcomes.

Because they get their demons out on paper and on film some of these guys may happen to be some of the most well adjusted people walking the earth.
