Pick two things that need to be done. One a sit down chore like homework, and one a stand up chore like clean your room. If you get restless on one, start the other. Keep going back and forth between them.

Drink lots of caffeine and play the radio while you perform household chores (not chorus) and get lots of rest/sleep when you're not doing all of these other things. It's hard to get motivated to do things that aren't pleasureable. I hate housework and procrastinate terribly.

Of course, you have the answers to your questions. I admit I'm amused with your identity.
I know who you are and I still like you.
Everyone I know (i've acquired many friends and acquaintances in 60 years) is lazy, procrastinating and agoraphobic to some degree ... so you are one member in a very large social group.
There are earthy sayings like, "no pain no gain," but, I bet you don't particularly care for anyone who brings it up (even yourself).
It's unfortunate for you to feel so worthless and unable to attend to these three, obvious, important activities in your life.
When I look closely at myself, I see me in you. I remember when I felt as you now feel, even though it's never really gone away. I seem to have grown in groups of 3. Every three years or so, I seemed to adjust and overcome, edging higher and higher to my goals ... working a little harder to make A's and B's instead of D's and F's, getting myself a little cleaner, making sure to contact those I considered friends more often.
Now I didn't overcome anything, my problems are still there, but, they have grown less influential as I have grown stronger in my attempts to improve them. I've watched others improve at a faster rate than me. As long as they didn't use their superiority against me I had no issue with them and they remained good friends.
For me to reach my goals, I had to apply myself to my immediate pursuits ... so I kept improving, but, it didn't come in a day. My period of adjustments occurred every three years, but, I did improve!
Although I'm middle-aged now I still get my good moods often and continue to improve.
I have no doubt you can see you in me.
You have the same answers I do and you have the same excuses. You know which ones to use to improve, no one needs to tell you what you already know.
Hum-m-m-m will you take 3 years to improve, or, 1 year ... maybe two? You'll decide when you get damned ready.
Best of luck!

What you term as Laziness could also be your personality... Some people just like to keep to themselves... I was like that during my highschool days.

I think friendly counselling will really really help... and obviously this has to be spread across a period of time.

Reject any responsibilities or obligations. You don't need that junk. In this way, you will have no reason to feel guilty.
