Since the cartoon didn't want to deal with the such issues as death it was really never asked or explained. However there are some interesting theories.
*Casper the motion picture said that casper feel victim of a childhood sickness. Probably Pnemonia.
* The Simpsons. Bart and Lisa were comparing each other's comic unfavourably, when Bart pointed out that Casper bore an uncanny resemblance to Richie Rich. Lisa then mused that perhaps Richie had become disillusioned with wealth, and took his own life.
* In the Family Guy movie, Stewie Griffin: The Untold Story, one scene depicts Casper's demise in an act accidentally initiated by Stewie.
* On Cheers, Norm Peterson observed that by the end of each episode Casper was surrounded by friends, but by the beginning of the next episode he was once again friendless. Cliff Clavin's response was that Casper was simply sating his appetite for human blood.
I know the last example was off topic but it was so funny I had to include it.
Also interesting is that in the first Casper cartoon Casper tries to make friends but they run away. Distraught, Casper unsuccessfully attempts to commit suicide (apparently forgetting that he's already dead) How do you forget this!!! I mean look at him its not like you can't notice your transparent?

How exactly did he die? The movie version made up that he got sick, but you never really knew for sure in the comic books.
Why isn't Casper in Heaven? As HOT STUFF shows you, the Harvey Comics universe clearly has a Hell, so what did Casper goof up to be stuck on earth for all eternity?

Well, in the movie Casper says that he stayed outside in the winter too long while he was sledding and got sick. Those aren't his real uncles, and I don't know how they die.

I anihilated them with my psychic powers. They deserved it, way too friendly. It was awesome, a great way to release repressed anger. Almost orgasmic.
