Purchase Order Form?
Of all the paperwork used in a real estate transaction, I have never heard of this form before.
If you reside in the U.S., your state has a Real Estate Commission or some government entity that issues licenses to agents and most likely will have state approved forms.
It is well worth it to pay a real estate attorney for a few hours of their time to protect you during the transaction.

don't be silly ,have an attorney draw on up for you.why risk a $200,000 sale over $125 attorney fee? just because you dont want to give an agent 4% of you sale doesn't mean you should skimp on an appraisal, proper paperwork, etc. it is all for your own protection.

If you don't use an agent, please consult an attorney. If you prepare the paperwork yourself without having a knowledgeable representative, you are making a big mistake.
