When you tell him you're feeling blue or down or bummed or whatever adjective you use for depressed.
Depression has no outward symptoms, as it is a psychological condition. It does manifest itself in lack of energy (fatigue), hyperactivity, loss of appetite or overeating, and other things that are typically out of character for that person. Of course these things are normally self-reported, since the doctor doesn't live with you and cannot observe you for an extended period of time.

Depression is visible in many ways. You might not have the proper weight gain, either over or under. You might have a blood pressure issue. Your appearance might be unkept due to the lack of caring for yourself.
If you are depressed, there is no reason to live like that. Your doctor is the best person to reach out to for help. I have been on antidepressants my whole pregnancy with the okay of my family doctor, my OB and the best high risk OB in the State of Florida. Please ask for help if you are feeling depressed, don't wait for someone to pick up on it, depression isn't a game!

Your doctor really doesn't spend enough time with you or know you well enough to notice it.
If you are feeling depressed, you need to discuss it with him or her. Let them know how you're feeling, how often, etc.
They can decide if it's hormones or depression.
It's nothing to be ashamed of either. About 20 percent of women suffer from depression during pregnancy, and most of them planned the pregnancies!

I remember when I was pregnant why nurse would ask certain questions every now and then and I think that is how they can tell if you are depressed or have something to worry about.

Well they don't know your personality so they don't just assume it. You usually have to express concern of depression. They will then evaluate you. I hope you feel better soon. For the sake of you and the baby.
