Yes, of course, you can. If live in the US, you just need to go to your local post office to get an application form. Fill out the application form, take two pictures of your 11 year old (the application form has descriptions on what kind of picture you need to provide), and get a money order. The application form has all the details that you need. Just follow the directions and your son will get a passport. It takes about 6 to 8 weeks to process unless you want express service.

Absolutely. The only thing to bear in mind is that passports for minors expire faster than passports for adults, I think because a minors appearance changes more dramatically in 10 years than an adults does.

Yes. You must have your childs birth certificate and you will have to have a picture for it taken (you can do this at the post office for a cost). You send the application to the proper authorities and it can take up to 6 months. If you need to get it sooner you will have to find the closes place to go and hand walk it through the process. There is also a cost for this as well.
