I'm not talking about semi trucks, but regular sized ones (Dodge, Ford, Chevy, etc.) They put out soooo much black smoke. Why do some states allow them to be exempt while other cars and trucks that run on gasoline have to be clean burning? Doesn't seem fair.

Because the black smoke you see is not anywhere near as bad as if it were invisible or white smoke. The black is carbon particulants that are bio-degradable. Gasoline exhaust expels terrible gases that are not bio-degradable and are loaded with elements that cause pollution.
Diesels are 94% cleaner in black exhaust than gasoline exhaust which you cannot see except on a cold day.

The reason is the prohibitive cost for diesel exhaust analyzers, starting in model year 2007 all diesels have federal OBD 2 emission standards, so their days of getting by without being tested are over
by the way diesels produce tons more Nox gases than gas vehicles so don't believe that they're environmentally friendly at all

In the state I live in it's a county by county thing. The county I currently live in does not require it. But the county I'm from does and so does many others.

Some states still don't require state inspections.

with time all states will have the test.that is in the u.s. states
