It means that you don't care what anyone thinks about you if they see it -- carefree. People are usually so self-conscious that even when they feel like throwing their hands in the air, they restrain themselves.

I believe that the just don't care refers to not being ashame to do it, since the idea that the ones who are not doing it may be concern about how dumb they willl look by trowing their arms.
