How could you find out if your phone is being tapped?

Get a high-end scanner. Put it next to the phone and have it scan all frequencies. When you hear a squelch you got one. Of course this is only for low-end cheap bugs.

Most taps nowadays are done away from the residence.

If you've discovered you're being tapped, those doing the tapping will now resort to more extreme measures in order to extract the information they need from you. I hope they haven't put a keylogger on your pc too.

You could ask your phone company, they might be able to tell. Or you could purchase one of those gizmos that detects phone taps on your line from any electronics store.
