Because Europe is being subducted under Africa, which is causing the African Plate to bend downward just before the collision zone (which is also called the Alps). Italy is just a higher part of the northern part of the African Plate, due to the mountain chain in the middle of the "boot" (the name of the chain eludes me right now). The fact that it looks like a boot is merely co-incidence.
