If you're using Internet Explorer go to Tools, Internet Options. Click Delete Temporary Internet Files, Delete Cookies, Delete History.
If you're using Firefox go to Tools, Options. Then click Clear next to History, Saved Form Information...
If that doesn't seem to work then go to the address bar and highlight a URL then hold down SHIFT + DEL.

Be sure to check your Favorites and Bookmarks too.

If you are using Windows 2000 or XP go to C:\Documents and Settings\User Account\Local Settings, User Account being whichever account you usually log on with, and delete all Temporary Internet Files and all History files. then back up to the C:\Documents and Settings\User Account folder and do the same in the Recent and Cookies folder. Other than wiping your harddrive and reinstalling, these are the most obvious. Try a program by Webroot called Window Washer. It's cheap and you can set it to do all that every time you boot up.
