Are you Canadian? Passports aren't renewed, you need to get a new one. You need to go and get a current passport picture taken and got to a post office or online and apply. I don't know why you would need your school records, that's crazy. As for your immunization, I didn't need that information for my passport either. however your doctor should have in his/her records your immunization history.
