In the absence of local tissue injury, the bleeding comes from the urethra which may be caused by infection, stones, vessel rupture, prostate problem or some kind of cancer. If such condition is not resolved shortly I advise him to see a doctor (urologist). The urologist will most likely perform some urine tests, x-rays and cystoscopy (looking inside the urinary passage with a small instrument) then you will know its real cause by then.

Obviously there is something VERY wrong. He may have an std or a medical problem that needs attention. Tell him to go to his doctor or a urologist and get checked out immediatley. If he has an std that means you may have contracted it from him and may need medical attention too.

If this is with ejaculation (hematospermia), it can be a benign condition without clear cause. It would be best to check it out with a urologist to rule out infection, etc.
