It just depends on which state you want to be licensed in. There are a few states that don't accept the NREMT results and insist on their own test. Most states will accept the NREMT certification, and it just takes a little paperwork to get your state license. In Alabama, all I had to do was take an Emergency Vehicle Operator Course (EVOC) and submit my NREMT certificate along with $10.00 to get my Alabama license.
It also depends on which level you are. For a paramedic, it can be a bit harder to get state certification. To get my Alabama paramedic license, I had to take a Transfer Drugs course and a Protocols course, too. In Alaska, you had to fill out a 1/2 inch thick stack of paperwork, get stuff notarized, and pay $150 to get an Alaska medics license. But in Alaska, medics are licensed by the same board that licenses doctors, so they are strict as heck. Most states are a lot easier. Also, most states require that you work for an ambulance service to get a license for that state. The best way is to run a Google search and type in your state name followed by EMS, and you will usually get the state board of EMS website. That will have all the info you need. If you can't do that, you can call the local ambulance service, and they will know how you can get state licensed.
