If a person's name is John Smith Jr., when putting the last name first, where should the suffix go? Should it be Smith, John Jr. or Smith Jr., John?

Does this work with other suffixes too? Is there something on the web that specifies the proper way to order names?

Last name first, followed by their first name, followed by any suffix.
ex:
Healthcliff, Clive Sr.
