Cradle cap (infantile seborrhoeic dermatitis)

Reviewed by Dr Paul Klenerman, consultant physician

What is it?

Cradle cap is a disease of the scalp in babies, characterised by greasy, yellow, scaly patches on the skin of the scalp. The condition is temporary and harmless.

Cradle cap is temporary and harmless.

What causes it?

The cause is unknown. It is not due to infection, allergy or inadequate washing.

What are the symptoms?

Greasy, yellow scaling patches that may, eventually, cause the baby's scalp to be covered in a thick, scaly layer. The condition is not itchy and the child is not distressed by it.

Who is at increased risk?

Cradle cap is very common in babies. It usually appears during the first couple of weeks of the infant's life.

>>>What can I do at home?<<<

The following treatment can be effective in controlling the problem, but it may have to be repeated. The baby can be made more comfortable by loosening or removing any clothing covering the scalp.

* Rub a little olive oil into the baby's scalp before bed. Comb the hair with a fine-toothed comb in the morning.

* When the scales have been removed, wash the baby's hair with a mild baby shampoo.

* If the treatment does not work, or if the baby has eczema on the face or body, consult your doctor or a pharmacist for advice.

How is cradle cap diagnosed?

The doctor will make the diagnosis by examining the baby's head. The appearance is usually quite characteristic.

What happens if it gets worse?

In rare cases, the condition can spread to the face, or to the body, where it can cause eczema in the nappy region (the groin) and the armpits.

Future prospects

Cradle cap is a temporary condition which will disappear by itself after a while.

What is the treatment?

The doctor will probably suggest a treatment similar to the one described above. If the baby has a severe attack, the doctor may choose to treat it with a weak hydrocortisone cream (eg Hc45), possibly in combination with a special shampoo.

Severe cases are usually referred to a skin specialist.

My four month old has had terrible cradle cap from birth. I also tried the olive oils and the baby oils and nothing seemed to work.

Finally his pediatrician recommended using Selsun Blue, which I thought to be a bit odd at first. I use it twice a week and within two applications his cradle cap is completely gone. Just be sure to keep it out of the eyes and out of the rest of the bathwater as it will irritate his/her skin.

Do not use olive oil on your baby! Olive oil is cooking oil. That'll just make it worse!

Use baby oil. Softly but generously rub it into the scalp. Use a fine baby comb, and gently comb thru the hair. Then, wash hair thoroughly with Johnson & Johnson's baby shampoo. I personally like the head to toe body wash - I don't have to buy tons of bottles.

Do this for a good couple days. If it doesn't get any better, then make an appointment with the pediatrician.

I only use Johnson & Johnson products. I find them clean, and gentle on sensitive skin!

My daughter had cradle cap when she was 3 months old. Her doctor prescribed a medicated shampoo called Ketoconazole 2%. Her cradle cap completely cleared up within one week and has never returned. She's going on 6 months now.

My son was a preemie and he spent a lot of time in the incubator and has this. One of the nurses at Children's hospital told me to use Head & Shoulder's...but to just make sure the baby is reclining so the shampoo doesn't get in their eyes. My son's cradle cap cleared up really fast using this.

It is dry skin, so when babies skin is very moist just scratch the top of her/his head. It will loosen and is relaxing for baby.

I never did put oil, lotion on my babies. Just use a regular type hair brush on babies head when you bath.

Every baby gets a touch of (cradle cRAp-as we call it) and the oils do not get rid of it, only softens the skin to help you remove it.

Try the scratching... if you do not have fingernails long enough, just use a comb, gently.

Just give it a good scrubbing. I think people are afraid to scrub because of the babies soft spot. Get in there and massage it with baby shampoo well, for about 4 minutes then rinse. This is what I did, and it worked.

When the baby gets their bath rub a little baby oil on their head brush it in a circular motion to help break up the dry skin, wash and rinse. Or you can ask your pediatrician they have a shampoo that is prescribed that you can try.
