The Guinness Book of World Records recognizes Nolan Ryan as having thrown the fastest clocked pictch in a major league baseball game. His speed was 100.9 MPH in a game against the Chicago White Sox on August 20, 1974.
Mark Wohlers was clocked by radar throwing 103 MPH in Spring Training in 1995. Joel Zumaya matched this in 2006 while pitching at the McAfee Colesium in Oakland.

It has been conjectured that troubled minor league pitcher Steve Dalkowski threw a fastball between 110 and 115 mph. in the late 50s and early. 60s. This was before radar guns and the speed of his pitches were of course estimated. One story connected with him has Dalkowski hitting and umpire in the mask with a pitch, breaking it in three places, and knocking the ump back eighteen feet. Needless to say he had a reputation for being 'wild' and never made it to the major leagues.
