all mammals descended from one creature...in this case one that had ten fingers, ten toes, 2 wrists, 2 elbows, 2 shoulders,2 ankles, 2 knees and 2 hips.
if a creature was born with one arm or leg (or even without a wrist, ankle, etc), it would be at a disadvantage, and would therefore die quickly, preventing it's defficient DNA from entering the gene pool.

The last common ancestor had that configuration. But many mammals have fewer than five fingers or toes. Horses have but one.

Uh, dolphins and whales don't have all those things, and they are mammals. But in answer to your question, the most effective body structure evolves, and the others get weeded out.
